A knock audible for the first few minutes after starting that dissolves as the engine warms is one of the most common engine stories. It is also the most deceptive: the same “cold” knock hides both a harmless design characteristic and the first signs of scoring in the bores. The key to telling them apart is not the sound itself but its duration, its character and its trend from month to month. Here is the whole list of causes in order, from the innocent to the serious.
The physics: where the knock goes
An engine is built from metals with different thermal expansion. While the engine is cold the clearances between parts are at their largest: an aluminium piston sits more loosely in an iron or steel bore, the valve clearances are wider, the oil is thick and slow to reach the top of the engine. As it warms the parts expand, the clearances converge on their working values, the oil thins — and the sounds born of enlarged clearances fall silent one after another.
Hence the rule: a knock strictly “when cold” is nearly always a story about clearance. The only question is which clearance is enlarged — by design or by wear.

Hydraulic lifters — small hydraulic supports that take up the valve clearance automatically — tick until they fill with oil, as set out in how to check hydraulic lifters. Cam phasers — the couplings that rotate the camshaft to change valve timing — rattle for a second or two at start-up while there is no oil pressure in them, cam phaser rattle when cold. A cracked exhaust manifold blows exhaust when cold and seals up as the metal expands — the sound is closer to a puff than a knock, burnt exhaust manifold gasket signs.
The harmless scenarios
You can breathe out if the knock is short and has been stable for years. The classic is a few seconds of lifter ticking after standing overnight: the oil has drained to the sump and for the first revolutions they work without a supply. The same goes for quiet muttering from the piston skirts on engines for which an enlarged cold clearance is a design feature — some engines clatter when cold from the factory. The signs of normal: the duration does not grow, the sound does not coarsen, oil consumption does not increase.

The worrying scenarios
Three signs call for attention. The first is the trend: the cold knock lasts longer and sounds coarser every month. The second is the company it keeps: oil consumption, blue smoke on a throttle blip, or rough running have joined it. The third is the character: not a soft tick from above but a dull diesel-like clatter from the block. The timing chain deserves a separate note: a rattle in the first one to three seconds after starting means the tensioner cannot tension the chain while there is no oil pressure. What to do and how the repair is decided
The first step is always the same and always cheap: check the oil level and recall the date of the last change. Fresh oil of the correct viscosity with a new filter resolves a noticeable share of “cold” ticking. If the knock remains, the order of investigation runs from measurements to parts: an oil pressure test, a scan tool read of the cam timing offset, and an endoscope into the bores. Only those three answers separate lifters from a chain and a chain from scoring — and each of them costs far less than the part it rules out.
Two things worth asking before agreeing to a big job. Which measurement led to the diagnosis, in numbers rather than impressions. And whether the oil history explains the wear: a fresh set of lifters or a new chain fed by the same neglected oil regime starts ticking again within a year.
